minor fixes to narration, crits/misses, thanks

On the narration side I have reduced the frequency of certain words appearing, like channeling, which wasn't accurate for enough contexts. I've also addressed some confusion with body parts, so one body part won't feed incomprehensibly into another body part. There was a sentence template or two that seemed to want to play into that kind of narration. They've been excised. There was also a small issue with switch-in narration where sometimes monsters would get confused about who or what they were talking about. That's been clarified for the LLM and should be addressed.

On the technical side, misses and critical hits can no longer trigger on the same turn.

I'm considering jamming a bigger LLM into this thing to see how it works and whether there is more free context for the LLM to conceptualise the characters it is supposed to be playing. It is very experimental and the methods I have used to put it together may not be the best for the job, but it is a subject I have a keen interest in, and the field as it stands is extremely undefined.
