Hi, friends!

Hotfix #4 is here. This time we took a particularly close look at story logic and quest states — the places where things used to behave… like a quantum object in the hands of a careless observer.

Patch Notes:

  • (Important!) Fixed the quantum uncertainty in Vlad the Younger’s fate — previously he seemed to remain in two states at once (oh, the irony)

  • (Important!) Fixed issues with the node at the end of Katerina’s story on Day 9 — it now appears consistently, and the quest can be canceled if it was completed unsuccessfully
Alas, we’re not Dankovsky ourselves, so this fix won’t repair saves for those who are already stuck in this state — it only prevents the issue going forward. If the problem has already happened in your playthrough, you’ll have to resolve it via the console (press Ctrl + Shift + ~): enter the command below and restart the day:

    set_global_variable Day9_Q1_MM_KaterinaIsDead true

  • (Important!) Fixed an issue with Oktay, the patient from Day 5. Unfortunately, if he has already bugged out in your playthrough, you’ll need to fix it manually.
To do that, open the console, switch to the Console tab, and enter the following command:

    set_global_variable Hospital_Oktay_MM_Diagnosed true

  • (Important!) Fixed interaction with Isidor’s body at the funeral. You still won’t win anyone’s favour for poking around thought

  • (Important?) Added a text hint to the save icon to show when the game is saving. You may have figured it out already, but it confused new players

  • Overall, fixed many logic issues around Saburov and Katerina. NB: the cabinet where she keeps morphine still isn’t highlighted — we know, we’ll fix it soon. You can still take it even when it’s not highlighted

  • Fixed a number of issues with quest “final thoughts” that prevented resets or showed incorrect outcomes: Isidor’s house (Day 2), Haruspex (Day 3), saving Nara

  • Fixed an issue with patients on Day 10 — an incorrect diagnosis could persist even after resetting the quest

  • Adjusted Rubin’s note on Day 10

  • Fixed the locking of Day 11 and Day 7 by key story events — they now lock as intended, and this is reflected on the Decree Board

  • Cathedral Caretaker ran off again! This time, at the start of Day 10 he was standing in the wrong spot and sometimes blocked the stairs. We brought him back and gave him a stern talking-to — third hotfix in a row he’s wandered off somewhere

  • Adjusted Andrey Stamatin’s behavior (or someone who looks an awful lot like him): returned him to the pub on Day 7, and taught him not to lean on thin air on Day 11

  • Fixed a premature dialogue with Georgiy on Day 9

  • Typos

See how many fixes there are now? That’s because you’ve been actively pointing out problems. Thank you — and please keep it up! If after the update you still run into bugs or oddities, report them in the appropriate Steam Community thread or the relevant Discord channel/topic.

Email reports are also extremely helpful: write to us at

and send the following files from your PC:

  1. Player and Player-prev logs located here:

  2. The problematic save file, located here:

Open the save profile folder — there are three of them: Default (slot 1), 1 (slot 2), and 2 (slot 3).

This helps us find the issue faster and fix it sooner!

Community25 Jun 2026Pain, Difficulty, Shabnak, and Other Topics: A Pathologic 3 UpdateHello everyone! This is Alexander Souslov, producer at Ice-Pick Lodge and game designer of Pathologic 3. The studio has been quiet for a while. We needed a moment to catch our breath. The team had no New Year holidays, and frankly, no vacations at all while working on the game's updates. But we weren't silent because we were lounging on a beach, eating watermelons and baursaks (if you've never…In-game event17 Mar 2026Update #6Hi, friends! Update #6 is out. This time we tuned several noticeable things at once: improved a bunch of Mind Palaces , made the interface more flexible (all the way up to toggling the HUD with a single button), added more menu sounds — and yes, we updated the door sound when leaving the Polyhedron ! On top of that, there’s a batch of logic, visual, and performance fixes, including improvements…In-game event2 Mar 2026Update #5Hi, friends! Update #5 is here. We continue to methodically knock the weirdness out of the game: broken decrees and busted quests, doors that shouldn’t be open, districts you suddenly can’t enter, and other adventures in system logic. And we’ve finally admitted the obvious: there’s nowhere to put the pistol and the Prototype, so they now honestly sit in their proper slots. Changes: · Glued the…
