Achievement Fixes

This update includes several fixes for achievements:

- Fixed several achievements not correctly working (Thanks DmitryShaburov!)
- Fixed a rare crash that could happen if a certain achievement was given in the first frame after a levelchange.
- Removed the "pacifist" achievement completely.

All achievements should now work as expected.
