Apple's combo trial displayed a slightly wrong input for the first Counter Hit Combo. Now the trial shows the right sequence;
Edited input remapping routine to use clearer button names (e.g. "Menu Selection Button" instead of "Select"
Changed the feedback form to display "feedback / contact form (online)" in the main menu
