- fixed some difficulty scaling inconsistencies
- added render occlusion mesh toggle
- tweaked accuracy scaling for low tier enemies
- fixed boss 2 not starting fight correctly
- fixed trigger bolt grab not working on pistols
- fixed rendering issue on low settings with muzzle flash
